DX.Forms

Part of the DataXchange NHS Suite

Overview

DX.Forms are a suite of electronic forms that cover staff changes within the NHS Trusts. Ensuring that the correct details are entered, are correctly authorised, and get to payroll/HR in a timely manner, DX.Forms have been shown to help reduce overpayments and emergency payments by a factor of 90%.

The forms currently available are:

  • Position Approval - a pre-form to authorise vacancy
  • New starter
  • Change form with financial implications
  • Change form without financial implications
  • Termination / leaver

We know that many Trusts has these or similar problems and the solution is a simple one, DX.Forms.

Each form displays all the information that was previously in a paper copy, with the ability to edit strictly controlled.

Notifications are emailed to each authoriser including a URL to link to document directly.

The record is sent to the next authoriser immediately and not lost on a desk causing cost and time to all involved.

If necessary the system can be set to make any or all fields mandatory.

DX.Forms gives simple, effective, control and reports for audit purposes are produced and checked monthly to ensure all forms are received and processed, this provides a backup check for any email reliability issues.

In addition, UHB have found that managers are now completing "paperwork" well ahead of time, to ensure that their budgets do not get affected, because everyone can be held accountable for their department and excess charges can be taken from individuals' budgets. Currently they have completed HR3 termination forms for staff 2 months ahead of time, ensuring managers are also doing their jobs properly.
